REGISTRATION

Registration for the ICE 2017 Annual Conference will be available at the online conference site through 5:00 p.m. (PT) on Friday, December 1, 2017. After that, you will need to register and provide payment onsite.

Registration Fees / NO INCREASE FROM 2016 PRICES!: The conference registration options and number of days attending, along with any associated fees, can be found below. The fees include registration, meals provided during the conference (continental breakfast, breaks and lunch) and conference materials*.



Discounted Registration Fees for Health Plans and Provider Organizations That Provided 2017 Funding for ICE:

In appreciation of their support, ICE is pleased to be able to offer registration discounts to health plan and provider organizations that contributed funds to the ICE 2017 operating budget. Please see below for specific information:

Those who are employed by the following organizations can use the "2017 Contributing Health Plan" registration options below (Aetna Health Care, Alameda Alliance for Health, Anthem, Brown & Toland Health Services, CalOptima, CalViva, Chinese Community Health Plan, Community Care Health Plan, Health Net, Humana, Inland Empire Health Plan, Inter Valley Health Plan, Kern Health Systems, Molina Healthcare of California, SCAN Health Plan, Sharp Health Plan, UnitedHealthcare)

Those who are employed by the following organizations can use the "2017 Contributing Provider" registration options below (Golden Empire Managed Care Medical Group, Imperial County Physicians Medical Group, Managed Care Systems, LLC, NAMM California, Scripps Physicians Medical Group, Southern California Physicians Managed Care Services, Inc.)



* Conference Materials: Speaker bios, presentations and session handouts will not be pre-printed and provided at the conference. A conference portfolio or bag will be provided with some documents related to the conference and print stations will also be available onsite at the conference.

Wi-Fi access will be available in the meeting space and ICE will once again be using a mobile conference app, which will include the agenda, speaker bios and session presentations, to provide a more interactive conference experience. There will also be a website associated with the app where materials can be downloaded just prior to the conference. Concurrent Breakout Sessions (Day 1 only; there are no breakout sessions on Day 2): If you are attending Day 1 of the conference, you will be required to enter breakout session choices as part of the online registration process. Please review the agenda under the Agenda tab prior to registering.

Payment Methods: Payment can be made by credit card or by check/money order. Checks or money orders should be made payable to Health Industry Collaboration Effort or HICE (Tax ID #90-0130332) and mailed to: ICE Registrar, 20 Eagle Claw Court, Reno, NV 89523. Please include your registration number on the check. If paying by check or money order, it is preferred that it be received within 14 business days of the date of the conference (by Tuesday, November 14, 2017), and it must be received prior to the conference (no later than Friday, December 1, 2017).

Confirmation/Invoice/Receipt: Upon completing the registration process, you will receive an e-mail confirmation from eventreg@iceforhealth.org. The confirmation notice contains information on how to access conference materials, so please review it. The e-mail confirmation can serve as an invoice if needed to submit to your accounting department when paying by check. If paying by credit card, in addition to the e-mail confirmation from ICE, a receipt from the ICE credit card processor, Authorize.Net, will be sent to the e-mail address provided for the billing information.

Refund/Substitution Policy: No refunds or credits will be issued once you have registered and paid, including if you registered under the standard registration option but were eligible for a contributing organization option. Organizations are allowed to substitute attendees for registrants who are unable to attend. Please notify the ICE Conference Coordinator of any substitutions in advance whenever possible.

Quantity Discounts: ICE does not offer quantity discounts for multiple attendees from an organization.

Waiver Statement: By attending the ICE Annual Conference, you give ICE permission to use your likeness and voice in ICE marketing materials and conference materials available for sale or for use on our website.
